Place the extracted .pkg file directly into the root of the USB drive (not inside a folder).

Power off the TV at the wall, then insert the USB drive into the USB 1 port. Initiate Update:

The indicator will blink slowly during the update. Once it blinks rapidly (or changes color), the process is complete.
